Corrossive (Acid) Ingestion Esophageal and gastric Injury

Suicide.... Please avoid that moment of decision.....

Suicide is the end of one's own body. Recently a very famous person committed suicide and the whole country was shaken. But do we know....? How many such cases happen around us every day. Such individuals who are involved in mental depression take different paths to the end of their body. In my career as a surgeon, many patients have come to me who have swallowed acid for suicide. I am writing about one such patient here. A 30-year-old woman, a loving husband and a five-year-old daughter is the perfect picture for a happy family. But in a trivial matter, she swallowed a bottle of acid in anger. The path from the esophagus to the stomach and intestine were permanently damaged. A decision taken in an instant and an entire family scattered. She was rushed to a hospital but her injuries were serious. The esophagus and stomach were removed by immediate surgery and a pipe for food was placed in the intestine. My efforts saved her but she got the life as a dependent. Even saliva could not be swallowed due to permanently damaged food pipe and stomach, so there was no question of food. The whole family was engaged in her service. She often had to be hospitalized for minor or major problems. The only good thing was that now desire to live was back in her. Seeing this, I (Dr. Dhaval Mangukiya) decided to create a new way for food. This surgery was very major and more intensive than the previous one. A portion of the large intestine was used to replace the esophagus for food passage. This very complicated surgery lasted for six consecutive hours. Multiple anastomosis was made from the throat to the abdomen to completely rebuild the passage of food. The only question in her eyes in the ICU and ward after the surgery was when will I eat by mouth? Finally four days after the surgery When I was confident about her healing allowed her to drink water and eat food. She was taking oral water almost after one and half years. Acids are readily available in our country. Unfortunately, I have been performing at least ten such surgeries every year but fortunately I have managed to make them smile again and I give full credit to the tireless efforts and patience of the families of these patients.

આત્મહત્યા એટલે પોતાના જ શરીરનો અંત. તાજેતરમાં જ ખૂબ જ એક જાણીતી વ્યક્તિએ આત્મહત્યા કરી અને આખો દેશ હચમચી ગયો. પણ આપણે જાણીએ છીએ.....? કે આવા તો કેટલાય કિસ્સાઓ આપણી આજુબાજુ દરરોજ થતા હોય છે. માનસિક હતાશા માં સંકળાયેલ આવી વ્યક્તિઓ પોતાના શરીરના અંત માટે અલગ-અલગ રસ્તાઓ અપનાવે છે. મારી સર્જન તરીકેની કારકિર્દીમાં કેટલાય દર્દીઓ મારી સમક્ષ આવ્યા જે આત્મહત્યા માટે એસિડ ગટગટાવી ગયા હતા. એવામાંનું જ એક દર્દીની વાત અહીં લખું છું. 30 વર્ષની યુવતી, પ્રેમાળ પતિ અને પાંચ વર્ષની બાળકી એટલે સુખી પરિવારનું સંપૂર્ણ ચલચિત્ર. પણ એક નજીવી બાબતમાં ખોટું લગાડી તેણીએ ગુસ્સામાં એસિડનો બાટલો જ મોઢે માંડ્યો. અન્નનળીથી લઇ અને સંપૂર્ણ જઠર સુધી નો રસ્તો કાયમીક રીતે ખરાબ થઈ ગયો હતો. એક પળમાં લેવાયેલો નિર્ણય અને એક આખો પરિવાર વેરવિખેર. તેણીને તાત્કાલિક દવાખાનામાં ખસેડવામાં આવી પણ પેટના દરેક અંગોમાં ઈજા ભયંકર હતી. તાત્કાલિક શસ્ત્રક્રિયા દ્વારા ખરાબ અન્નનળી અને જઠરનો નિકાલ કરી ખોરાક માટેની પાઇપ આંતરડામાં મૂકવામાં આવી. મહામહેનતે તેને બચાવી તો લેવામાં આવી પરંતુ હવે પછીનું જીવન એક આશ્રિત તરીકે જ મળ્યું. ખરાબ રસ્તાને કારણે લાળ પણ ગળા નીચે ઉતારી શકાતી ન હતી તો ખોરાકનો તો સવાલ જ નહોતો. આખો પરિવાર તેની સેવામાં લાગી ગયો હતો. નાની-મોટી તકલીફો માટે પણ વારંવાર તેને હોસ્પિટલમાં દાખલ કરવી પડતી હતી. સારી વાત ખાલી એક જ હતી કે હવે ફરીવાર જીવવાની ખેવના પાછી આવી હતી. આ જોઈને જ મેં (ડો.ધવલ માંગુકિયા) એ ખોરાક માટે નવો રસ્તો બનાવવાનો નિર્ણય લીધો. આ શસ્ત્રક્રિયા આગળ થયેલ તાત્કાલિક શસ્ત્રક્રિયા કરતા ખૂબ મોટી અને મહેનત માંગી લે તેવી હતી. આંતરડાંના એક ભાગનો ઉપયોગ કરી ખોરાકના રસ્તા માટે અન્નનળીની જગ્યાએ મૂકવામાં આવ્યો. ખૂબ જટિલ એવી આ શસ્ત્રક્રિયા લગાતાર છ કલાક ચાલી હતી. સંપૂર્ણ રસ્તાના પુનઃનિર્માણ માટે ગળાથી લઇને પેટ સુધી આંતરડાંઓના બહુવિધ જોડ બનાવવામાં આવ્યા. શસ્ત્રક્રિયા બાદ આઈ સી યુ અને વોર્ડમાં તેની આંખોમાં એક જ પ્રશ્ન હતો કે ક્યારે મોઢાથી ખાતી થઈશ? આખરે શસ્ત્રક્રિયાના ચાર દિવસ બાદ મને સંપૂર્ણ રૂઝનો વિશ્વાસ આવ્યો ત્યારે પાણી પીવાની અને જમવાની પરવાનગી આપી. પાણીનો આ ઘૂંટડો આશરે દોઢ વરસ પછી તેણીએ ગળા નીચે ઉતાર્યો હતો. એસિડ આપણા દેશમાં સરળતાથી ઉપલબ્ધ છે. કમનસીબે દરેક વર્ષે હું આવી ઓછામાં ઓછી દસ શસ્ત્રક્રિયા કરું છું પણ નસીબજોગે એમને ફરી હસતા કરવામાં સફળ પણ થયો છું અને તેનો સંપૂર્ણ શ્રેય હું આ દર્દીઓના પરિવારજનોના અથાગ પ્રયત્નો અને ધીરજને આપું છું.

Choledochal Cyst: Dr Dhaval Mangukiya Gastro Surgeon

Every disease is a story of struggle which brings success....

A young 19 year old boy entered my consulting room with despair but hope in his eyes. His suffering was not of hours or days but of years. I could see scars of two old major surgeries as if someone had made a map on his abdomen. When he was 5 years old, his parents found that he had a disease in which he had dilatation of bile duct which causes jaundice. In medical language it is called Choledochal cyst. Even the surgeons do not know much about this disease and its surgery during those years. He underwent surgery and survived, but at 14-years difficult sufferings began. The disease was not completely cured but symptoms such as pain, recurrent jaundice, and fever were also found as gifts. Then a second emergency surgery was performed but the surgeon was unsuccessful due to prior surgery and inflammation in the pancreas. Now they were sitting in front of me with tears in their eyes and heavy despair. This time also he came with swelling in pancreas and jaundice. So I told them not to hurry for surgery. It was very difficult for me to explain that it's better to wait for some time. With the sum of my patience and minus of their haste I planned his surgery. Arriving at the operation theater for him, half the battle was won as many surgeons were not even ready to touch this patient in past years. But for me the battle was just the beginning. Like any other war, the preparation was sufficient, but the disease was to be faced as the soldiers would fight against the enemies in the desert. Due to inadequate prior surgery, organs within the abdomen were stuck in the upper part. Taking care of all these organs and reaching the main disease was like making a mine. Eventually the way was found but due to repeated infections and jaundice, the bile duct and the veins of the liver around it got stuck as if someone had put glue on it. After very careful and precise work, a diseased bile duct was finally removed and a new bypass was made for bile. The surgery lasted 4 hours and the result of this hard work was that the 14-year distress was finally cured within 4 days after the surgery. The young age of the patient and the care of his family helped a lot. Now when he came back into the consulting room there was a twinkle in his eye instead of sadness or despair and the happiest smile on his face and family.

19 વર્ષ નો એક યુવક મારા કન્સલ્ટિંગ રૂમ માં નિરાશાના ભાવ સાથે દાખલ થયો. તેની તકલીફ કલાકો કે દિવસોની નહિ પણ વર્ષોની હતી. પેટ ઉપર જાણે કોઈએ નકશો બનાવ્યો હોય તેવા જુના બે મોટા ઓપરેશનોના કાપા જોઈ શકાતા હતા. માત્ર 5 વર્ષ ની ઉંમરનો હતો ત્યારે તેના માતા પિતાને માલુમ પડ્યું કે તેને પિત્ત ની નળી પહોળી થવાની બીમારી છે. જેને કારણે કમળો થતો હતો. અમારી ભાષામાં એને કોલેડોકલ સીસ્ટ (Choledochal cyst) કહેવાય છે. સર્જનગણમાં પણ એ વર્ષોમાં આ બીમારી અને તેના ઓપેરશન વિષે આટલી જાણકારી ના હતી. એ વખતે તેનું ઓપેરશન કરવામાં આવ્યું અને મૃત્યુથી તો બચ્યા પણ ત્યાંથી શરુ થઇ 14 વર્ષની યાતનાની શરૂઆત. સંપૂર્ણપણે આ બીમારીનો રસ્તો તો ના જ નીકળ્યો પણ દુખાવો, વારંવાર થતો કમળો અને તાવ જેવા લક્ષણો ભેટ માં મળ્યા. બીજું કટોકટીનું ઓપરેશન કરવામાં આવ્યું હતું પરંતુ અગાઉની શસ્ત્રક્રિયા તથા સ્વાદુપિંડના સોજાને કારણે સર્જનને સફળતા ના મળી. હવે આંખોમાં ભય અને ભારોભાર નિરાશા સાથે તેઓ મારી સામે બેઠા હતા. આ વખતે પણ સ્વાદુપિંડમાં સોજો અને કમળો જાણતો હતો. આ કારણે મેં તેમને ઓપેરશનની ઉતાવળ ન કરવા જણાવ્યું. તેમને સમજાવવા ખુબ મુશ્કિલ હતા કે રાહ જોવામાં જ ભલાઈ છે. મારી ધીરજનો સરવાળો અને તેમની ઉતાવળની બાદબાકી સાથે એક સમયે પૂરતી તૈયારી કરી તેની શસ્ત્રક્રિયા રાખવામાં આવી. તેમના માટે ઓપેરશન થિયેટરમાં આવ્યા એટલે અડધી જંગ જીતાઈ ગઈ હતી કેમ કે આટલા વર્ષોમાં કેટલાય સર્જનોએ આ દર્દીને હાથ લગાડવાની પણ ના પડી હતી. પણ મારા માટે જંગ હવે શરુ થતી હતી. અન્ય કોઈ યુદ્ધ ની જેમ જ તૈયારી તો પૂરતી હતી પણ રોગ નો સામનો જેમ સૈનિકો રણમેદાનમાં દુશ્મનોનો કરે એમ થવાનો હતો. અપૂરતી પૂર્વ શસ્ત્રક્રિયાને કારણે, પેટની અંદરના અવયવો ઉપરના ભાગમાં અટવાઇ ગયા હતા. આ બધા અંગોની કાળજી રાખી મુખ્ય રોગ સુધી પહોંચવું એ જાણે ખાણ બનાવવા જેવું કઠિન હતું. આખરે રસ્તો મળી ગયો પણ વારંવાર ચેપ અને કમળો થવાને કારણે પિત્તની નળી અને તેની આજુબાજુની યકૃતની નસો પણ જાણે કોઈએ ગુંદર લગાવી હોય તે રીતે ચોંટી ગઈ હતી. ખૂબ જ સાવચેતીભર્યા કાર્ય પછી, આખરે રોગગ્રસ્ત પિત્તની નળી દૂર કરવામાં આવી અને પિત્ત માટે એક નવો બાયપાસ બનાવવામાં આવ્યો. 4 કલાકની મહેનતનું પરિણામ એ આવ્યું કે 14 વર્ષની તકલીફનું નિવારણ આખરે શસ્ત્રક્રિયા પછીના 4 દિવસમાં થયું. દર્દીની યુવાન વય અને તેના પરિવારની કાળજીનો મને સહકાર મળ્યો. હવે ફરી પાછો જયારે એ કન્સલ્ટિંગ રૂમમાં આવ્યો ત્યારે દુઃખ કે નિરાશાના સ્થાને આંખોમાં ચમક હતી.

Gastro Intestinal Tuberculosis

T.B.(Tuberculosis) is a very common disease in our country. Incidence of pulmonary TB is highest. The information about this disease that most people have are symptoms like coughing and bleeding in the sputum. But it's not true for abdominal tuberculosis and many times remains silent. Similar situation happened with this 22 year old male where he had been suffering from abdominal pain for three months. He visited a few doctors but the diagnosis could not be cracked. Which is often difficult to do due to intestinal complications. The trouble was growing, his well-wishers were upset and there was no redress. As the abdominal pain became unbearable, the super specialist Dr. Dhaval Mangukia was contacted and it was found out that he was diagnosed with abdominal TB. Due to this, the intestinal tract became constricted and this caused obstruction with bloating in the intestines and even ruptured from one place. Consent to surgery at such a young age was like cutting off a wrist for his parents. After adequate persuasion and taking the patient into confidence, the infection spread throughout the abdomen was cleared by surgery. The internal condition of the abdomen was so severe that intestinal repair was not possible. For which intestinal bypass (stoma) was resorted to. The first step was to avoid a fatal situation. The challenge now was to cure TB and reconnect the gut. Intestinal attachment was also performed after the condition was brought under control and T.B. Was also completely cured. The hardships of six months of life were a game of luck but the life that followed was the result of the hard work of doctors and their families.

આપણા દેશમાં ટી.બી.(ક્ષયરોગ) ખૂબ જ સામાન્ય એવી બીમારી છે. જેમાં સૌથી વધારે છાતીનું ટી.બી. જોવા મળે છે. મોટાભાગના લોકોને ટી.બી. એટલે ખાંસી આવવી, ગળફામાં લોહી આવવું જેવા લક્ષણોની માહિતી હોય છે. પણ જ્યારે પેટનું ટી.બી. થાય ત્યારે આમાંના કોઈપણ લક્ષણો જોવા મળતા નથી. આવી જ રીતે પેટના ટી.બી.થી અજાણ એક ૨૨ વર્ષીય યુવાનને ત્રણ મહિનાથી પેટમાં દુ:ખાવાની તકલીફ થતી હતી. ઘણાં ડોક્ટરો ઘણા બદલાયા પણ નિદાન પાકું થતું ન હતું. જે આંતરડાની જટિલતાને કારણે ઘણી વખત કરવું મુશ્કેલ છે. તકલીફ વધતી જતી હતી. તેના હિતેચ્છુઓ પરેશાન હતા અને નિવારણ થતું ન હતું. પેટનો દુ:ખાવો અસહ્ય થતાં સુપર સ્પેશિયાલિસ્ટ ડો.ધવલ માંગુકિયાનો સંપર્ક કર્યો અને નિદાન થતા માલૂમ પડ્યું કે તેઓને પેટના ટી.બી. હોવાને લીધે આંતરડાંનો ભાગ સંકોચાય ગયો હતો તથા આ જ કારણે આંતરડામાં અટકાવો વત્તા ફુગાવો થયો હતો અને એક જગ્યાએથી ફાટી પણ ગયું હતું. આટલી યુવાન વયે શસ્ત્રક્રિયાની સંમતિ આપવી એ તેમના માતા-પિતા માટે જાણે કાંડા કાપી દેવા બરાબર હતું. પૂરતી સમજાવટ પછી અને દર્દીને પણ વિશ્વાસમાં લઈને આખા પેટમાં ફેલાયેલો ચેપ સાફ કરવામાં આવ્યો. પેટની આંતરિક પરિસ્થિતિ એટલી બધી ગંભીર હતી કે આંતરડાનું જોડાણ પણ શક્ય ન હતું. જેના માટે આંતરડાના બાયપાસ (સ્ટોમા)નો સહારો લેવામાં આવ્યો. પહેલો પડાવ એટલે કે જીવલેણ પરિસ્થિતિ ટાળવાનો પાર પડયો. હવે પડકાર હતો ટી.બી.ના ઈલાજનો અને આંતરડાને ફરી પાછા જોડવાનો. પરિસ્થિતિ કાબૂમાં આવ્યા બાદ આંતરડાનું જોડાંણ પણ કરવામાં આવ્યું અને ટી.બી. નો સંપૂર્ણ ઇલાજ પણ કરવામાં આવ્યો. જીવનના છ મહિનાની મુશ્કેલીઓ એ ભાગ્યની રમત હતી પરંતુ ત્યારબાદનું જીવન ડોક્ટરો અને તેના પરિવારજનોની પરિશ્રમનું પરિણામ હતું.
