Transperitoneal Necrosectomy
45 year old female with history of idiopathic acute necrotising pancreatitits
Spent more than one month for severe attack with multi organ failure
Had tracheostomy and two pigtail insertion for infective collection
Recovered well from organ dysfunction
She had persistent fever and pus discharge from both drains
CT scan done after two and half months of attack
Findings suggestive of multiple necrotic collection & splenic abscess
Spleenic flexure encased in tail pancreas fibrosis leading to obstruction
Open Transperitoneal necrosectomy was done with diverting ileostomy.
Postoperative stay in ICU for 1 day and full diet started.
She had residual 20 ml collection in spleenic fossa
Pigtail insertion done for same and she recovered well.

Large Lower CBD stone sized 1.1cm
Attempted ERC but failed to remove
Operated Laparoscopically

Right Hemicolectomy in complete Malrotation
88 year old lady with ECOG status 0
Presented with bleeding PR
CT Scan and Endoscopy diagnosed Carcinoma of caecum
She had complete Malrotation with whole colon in left
Right hemicolectomy done

Mesenteric Vascular Ischaemia
Patient with Acute Mesenteric Ischaemia due to SMA thrombosis.
Bowel resection with thrombectomy using fogarty catheter
and relook exploration after 24 hrs showed reperfused ischaemic bowel.
Proximal ileal stoma and distal feeding made both side of incision
ICU stay with ventilator support 4 days
Discharged on post op day 8 with refeeding

Laparoscopic sigmoid colectomy
60 year old lady with history of discharging sinus in lowe abdomen
History of I & D for small abscess 11 months back in hypogastric region
Which later developed as pus discharging sinus
On CT scan she had complicated sigmoid diverticulitis with fistula
Colonoscopy confirmed diagnosis
Operated for Laparoscopic Sigmoid colectomy
Post operative period was uneventful

28 year old female with history of pain in abdomen
diagnosed to have hydatid cyst in segment 6 and 7 of liver
Given 2 weeks course of Tab Albendazole
Operated for laparoscopic drainage, lavage
Post operative recovery was uneventful
Patient discharged on post op day 2
